Register Today in Final Forms

FinalForms is NOW OPEN for sport registration and form completion for the 2025-2026 school year. Instructions are below on how to log in and indicate the sport student-athletes are interested in pursuing this coming school year.

Important: There are forms that the student-athlete needs to sign, along with a parent, for each sport.

Our WC Coaches use FinalForms to see who is interested in their team and to communicate with them important information about the upcoming season.

FinalForms must be completed BEFORE a student-athlete can participate in ANY sports. The parents and student athletes need to log in and sign the forms, and a current physical must be on file to participate.

FINALFORMS INSTRUCTIONS:

  1. Here is the FinalForms link https://worthingtonchristian-oh.finalforms.com
  2. Please either drop physicals off in the office or even better scan and email them directly to lynn.hartings@worthingtonchristian.com or to the Athletic Trainer, Amanda Moskal at amanda.moskal@worthingtonchristian.com . Parents can’t upload physicals to FinalForms. The Athletic Trainer checks them herself to ensure the athlete is cleared for participation in their sport. 
  3. Once student-athletes are in FinalForms, they can be added to a specific sport. 
  4. Student-athletes must complete the student signatures on FinalForms. They must log in to FinalForms using their Worthington Christian email and password to sign the forms virtually. An athlete is NOT eligible to participate if their student signatures or parent signatures aren’t complete.  
  5. An athlete must be completely cleared in final forms before they can participate in a tryout or practice. 
  6. You can change sports selected at any time throughout the year.

6th Grade Offerings

When there is a need due to low participation, we will open Middle School Sports to 6th graders who would be interested in trying out. As for now, for this upcoming year, the following sports are open to students who will be 6th graders in in Fall of 2025 to tryout:

  1. MS FOOTBALL
  2. MS GOLF
  3. MS GIRLS TENNIS
    SPORTS PHYSICALS

    These are also a requirement each year for student athletes.

    You can submit your physical:

    Families may not upload their physicals into FinalForms themselves. These must go through the WC athletic office to be reviewed and placed on file by our Athletic Trainer before the athlete is cleared for participation.

    UPCOMING DATES

    Fall Sports Parent Meeting | July 29 | 6:30 PM | Upper School
    At least one parent is required to attend this meeting with the participating student athlete.

    Fall Sports Tryouts | July 31 – August 1
    Some fall teams will hold optional training and team events throughout the summer.

    Fall Sports Photos | Starting August 6
    Team, individual, and senior banner pictures. More information and schedule to follow.
